Pseudo-Vestibular Neuritis Associated with Isolated Vestibular Nucleus Infarction

Abstract
Acute peripheral vestibulopathy (APV) is characterized by acute prolonged vertigo with unidirectional spontaneous nystagmus and positive head impulse test without any other neurological signs. Although APV is mainly ascribed to viral inflammation of the vestibular nerve, some lesions affecting pons or cerebellum can mimic APV, so called pseudo-vestibular neuritis. We report a 78-year-old woman with an isolated vestibular nucleus infarction who presented with acute onset of prolonged vertigo. The clinical features resembled vestibular neuritis. Isolated vestibular nucleus infarction should be considered in the differential diagnosis of central vestibular syndrome mimicking APV.
